After you receive your doll and are ready to submit it to the project, you will also need to download and complete all required information on this Doll Information form and this Release Waiver form. THESE ITEMS MUST BE SENT IN WITH YOUR DOLL for it to be able to join us. We hope this is not an inconvienence for you.
